Melasma – a condition that needs the right diagnosis

Melasma is a chronic hyperpigmentation condition, usually appearing symmetrically on both cheeks, the forehead and the bridge of the nose. Its formation involves several factors: genetics, hormonal change (especially after pregnancy), sun exposure, prolonged stress, skin inflammation and sometimes the use of unsuitable cosmetics.

Clinically, melasma is classified into three main types:

  • Epidermal: pigment concentrated in the epidermis, with clear borders, usually responding well to treatment
  • Dermal: pigment deep in the dermis, grey-blue in colour, harder to treat
  • Mixed: a combination of both – the majority of real-world melasma cases, especially post-pregnancy melasma

The core difference of E-Mela is its ability to act on deep-layer pigment. At Eternal, a complete pigment protocol works on two levels:

  • Pigment peel: removes the thickened keratin layer, smooths the surface and treats melanin at a depth below 200 cell layers – suited to superficial pigment

    The dermatologist examines you directly, assessing the melasma type (epidermal, dermal, mixed), the depth and density of pigment, your melasma treatment history, the state of the skin barrier and any relevant hormonal factors. On that basis, the doctor builds a personalised E-Mela protocol for each client.

    Cleansing and skin prep

    The skin is cleansed gently and the base is strengthened with recovery and regenerating serums before the intensive actives are applied. This helps the actives absorb evenly and limits unnecessary irritation – especially important for the sensitive skin base often seen in long-standing melasma clients.

    Superficial pigment peel

    The pigment peel removes the thickened keratin layer, smooths the surface and acts on melanin at a depth below 200 cell layers. This is the foundation step that helps E-Mela absorb more deeply in the next step.

    The E-Mela technique

    Resorcinol is zoned according to the melasma map defined during the consultation. Darker, long-standing areas are treated more thoroughly, focusing on deep pigment beyond 200 cell layers – where ordinary peels cannot reach. This entire step is performed under dermatologist supervision.

    Soothing and recovery

    After the E-Mela step, the skin is soothed with a recovery mask and dedicated serum. This helps reduce any mild stinging and reinforces the skin barrier.

    Finishing and aftercare guidance

    The skin is finished with moisturiser and sunscreen. You receive specific aftercare guidance for the first 5–7 days. Depending on the melasma and the protocol, the doctor may prescribe home-care products to maintain results between in-clinic sessions – melasma is chronic, and continuous care between sessions plays an important role in long-term results.

How is E-Mela different from laser melasma treatment?

After the first session, the melasma usually fades slightly, the skin tone looks more even and the barrier visibly improves. Some clients may feel mild stinging or slight redness in the first 6–24 hours – a common reaction that settles with proper care.

For resistant, long-standing or post-pregnancy melasma, Eternal typically recommends a course of 4–8 sessions spaced 2–4 weeks apart as advised by the doctor. Once the melasma stabilises, the in-clinic interval is extended to maintain results.

An important note: melasma is a chronic condition that can recur when triggers occur (sun, hormonal change, unsuitable cosmetics). E-Mela helps improve pigment and even out skin tone, but it is not a "permanent cure". Long-term results depend on the melasma type, how closely you follow the protocol and, above all, daily sun protection.

After your session

  • Strict sun protection – use SPF 50+ sunscreen daily, reapplying every 2–3 hours outdoors. This is a mandatory step and the single most important part of any melasma protocol.
  • Wear a wide-brimmed hat and a sun mask, and avoid going out during peak sun hours (10:00–15:00).
  • Follow the home-care products as directed by the doctor – do not stop midway through the course.
  • Pause retinol, AHA, BHA and strong treatments for the first 5–7 days (unless the doctor advises otherwise).
  • No makeup for the first 24 hours after the treatment.
  • Limit saunas, intense exercise and hot showers for the first 48 hours.
  • Moisturise and support the skin barrier fully.
  • Do not perform peels, microneedling or other procedures at home throughout the course.
  • Contact Eternal if anything unusual appears, such as prolonged redness, swelling or increasing irritation.
